New Patient Registration

We welcome new patients who reside within our practice area.

When regsitering at St James's we allow 72 hours to register you once we receive your registration papers. After 72 hours you can make an appointment.

The map shows the contractual boundary for St James's Surgery (in green). Anyone living within this boundary is able to register with St James's. If you are already registered with us and move to an area outside of the green boundary it may be in your best interest to find a practice closer to your new home. However, we will retain, where appropriate, existing patients who have moved house within the green boundary into the outer boundary area (shown in purple). Where a patient moves from the existing practice area (green) to the outer boundary area (purple) and remains registered, we will continue to provide the full range of services to you, including clinically necessary home visits. However, if you rely on frequent home visits from us, it would normally be in your best interest to choose a practice closer to your home. 

Recently will have joined a GP Scheme to keep patients registered or take on new patients if they are out of area. You will need to inform reception in person of these changes and we will create a pop up on your notes to say you are 'Out of area', the only service we wouldn't be able to offer is home visits, so if you were very poorly and couldn't physically make it into the surgery you would need to ring 111 to request a GP visit.

We do advise if you are on long term medication which needs regular check up's or housebound to register with a practice near your home address.

Registration forms are available from reception or you can download them via the links below. Please complete and return to the surgery. Please note: You will need to bring photographic ID & proof of residency to the surgery to complete your registration. This MUST be seen before registration.
